• Customer’s / contact person’s name
• Organisation’s name
• Address, telephone, fax and email address
• Source language (the language the document is in)
• Target language (the language you want it translated into)
• Brief description of document
• Number of words
• Date by when translation is required

Please attach the document if possible.
